Dedicated Diabetic Clinic at Catherine Street Medical Centre
Diabetes currently affects 246 million people worldwide and is expected to affect 380 million by 2025. There are currently over 2.5 million people with diabetes in the UK and there are more than half a million people with diabetes who have the condition and don't know it. Even though diabetes affects nearly 4% of the world’s population, many people know very little about the disease.
Even though Diabetes affects nearly 4% of the world's population, many people know very little about the disease.
We run a dedicated Diabetic clinic, at which we monitor Diabetic patients' blood sugar control & we strive to reduce their risk factors for heart disease and the complications of Diabetes.
Ultimately, our goal is to improve the quality of life of our Diabetic patients.
Dedicated Heart Watch Clinic
For patients with established heart disease, such as heart attacks or those with coronary stents inserted, this prevention programme assists in secondary prevention of coronary heart disease (CHD).
The specialist nurse moniters blood pressures, cholesterol, smoking status, exercise, and weight changes (BMI) on a regular basis.
The Heartwatch programme has demonstrated significant improvements in reducing risk factors and improving our treatment of heart disease.
Care of the Elderly Clinic
In recent years we have established a special service for our older patients.
This is free of charge to medical card patients and costs a normal consultation fee for private patients.
The goal of the clinic is to be proactive in seeking ways to assist this potentially vulnerable group of patients.
This involves a Practice Nurse led assessment of our elderly patients and includes a review of patients compliance with their medications, their level of dependence or independence at home, their risk of falling, their need for physiotherapy to reduce the risk of falling.
Our Nurse establishes how the patient is performing with respect to mobility, independance, safety etc. and considers with the patient any further help that we might be able to source eg Public Health Nurse , Occupational Therapist , Eye review , Hearing test, Personal Alarm system etc.
If the Nurse is concerned with any of these issues she arranges assessment by the Doctor.
Blood tests are taken if due and an ECG (Heart tracing) as well as weight , blood pressure and a follow up appointment made with the Doctor if required.
This Clinic has proven extremely popular with our patients and very useful to the Doctors in maximising our information and potential to help this valued group of people.
